Prior to your visit, we will have a complementary phone consultation to address any needs and questions you may have. We will also schedule an in office consultation and evaluation at this time. After you are welcomed into our eco-friendly office, our friendly team will assist you with intake forms at the front desk. During your consultation, we will review your personal history, background, symptoms and concerns. It is helpful if your are open and honest with me so that I can better assess your situation. At the end of the assessment, we will work together to create a manageable follow up plan. I will also give you any helpful recommendations. This appointment takes approximately 90 minutes, which includes time for intake forms and scheduling.
You understand that you are responsible to keep your counseling appointments, and that you are to notify the counselor at least 24 hours in advance of any appointments which cannot be kept. If you provide less than 24 hours notice, your account will be charged for the regular rate of your usual appointment.
